The following information is provided as a disclosure to you as a “User” of this service regarding basic details about how this works and what you can expect. If you desire more detailed information do not hesitate to contact us.

SERVICE PROVIDERS. Users will not initially know the exact name or exact location of a testing center where the test is performed. They will be aware of this after purchase. The test services are tied to the geographical area chosen by the user.  Medications are mailed using the mailing address provided by the User.  Users that bid on the price of the service understand that their purchase is non-refundable. Those who choose the” Buy Now” option may have their purchase price refunded if not used within 30 days,  minus a $30 administration fee.

INSURANCE  REIMBURSEMENT.   BidOnHealth.com does not submit claims to insurance companies for re-imbursement. That is the User’s obligation if insurance reimbursement is available. While our website operates on a cash only basis, users may be able to submit expenses incurred on BidOnHealth.com for reimbursement from a health savings account or apply to a deductible to user's insurance policy. It is the User's responsibility to contact their insurance company to see if these medical costs will apply to their deductible.

“LOWEST PRICE” OPTION.  User understands and agrees that if the "Lowest Price" option is chosen as an example - for the Greater Phoenix area - User may have to go outside of their personal geographic area to obtain the lowest price. For example, a person living in Glendale, Arizona who wants the best price on a CT scan may be required to drive to Mesa, Arizona to get it.

USE OF  VOUCHERS.  User is informed and understands that a voucher is transferable but expires thirty (30) days after purchase. User understands they may choose to purchase a test or medication that is different than what their doctor ordered.  This may or may not result in a price change and/or delay in service. The item or service must always match exactly what the health care provider has ordered.

ERRORS.  BidOnHealth.com is not responsible for errors in purchasing goods or services which are made by Users.  BidOnHealth.com is not responsible for any errant actions, inactions or medical errors caused by or created by User or any of our affiliates.

PHARMACEUTICALS . User understands that the medications they are purchasing are primarily generic versions that are mostly manufactured outside of the USA in FDA approved manufacturing facilities and facilities that meet the World Health Organization’s GMP "Good Manufacturing Practices". standards and certification requirements. They are not “brand name” medications. A pharmacy we use is based in India and supplies large networks of hospitals there with the same medications. In 1999, the United States FDA stated “ generic medications are chemically equivalent to brand name medications at a substantially lower cost”.

PRESCRIPTIONS REQUIRED – PROCEDURES. Users must either provide a digital photo or faxed prescription from their doctor (standard forms are available for easy ordering). Users can either fax the prescriptions or e-mail a digital photo to the number listed on the voucher we provide. User understands that shipping times may take up to 20 business days to arrive. They need to plan accordingly. Many medications are received in about 7-10 business days and are shipped directly to the User’s mailing address. A shipping charge of $7 is added to medication orders.

RADIOLOGY  SERVICES. User understands they are purchasing a specific time slot for a radiology test and therefore are responsible for keeping the appointment. If a time slot goes unfilled the radiology group may, at their discretion, not reschedule due to down time of the machine caused by user’s failure to keep the appointment.  User understands that all radiology centers utilized by BidOnHealth.com are fully licensed by the relevant state, accredited by the American College of Radiology, and only board-certified radiologists interpret the films. Once a test is purchased, the radiology clinic will contact the User to set up an appointment. The User must take the doctor's order and the voucher to the appointment which will be redeemed at the place of service.

LABORATORY SERVICES. User understands that when purchasing a laboratory test such test will be conducted at a certified lab which are located based throughout the geographic area. Tests are performed at varying times during each day. These times for your test may be during “off-peak hours” and may therefore be during the middle of the day. User therefore understands you may not be able to use the voucher during “peak hours”. User further understands that lab centers utilized are fully licensed and accredited to perform the lab procedures.

II.TERMS OF SALE

All vouchers printed from the Site, or any other website associated with BidOnHealth.com (hereinafter “Vouchers”) are promotional vouchers that may be purchased from participating providers (“Providers”) through BidOnHealth.com to be used in exchange for goods and services at a discount from their actual cost. By placing an order for a Voucher from a Provider through BidOnHealth.com, you make an offer to purchase the Vouchers you have selected on the terms and conditions stated below. For more information about our collection of personal information, please view our Privacy Policy. You are required to create an account in order to purchase any Voucher. This is required so we can provide you with easy access to print your orders, view your past purchases, modify your preferences, and to ensure permissible use of the Voucher.

The Voucher you purchase through BidOnHealth.com is redeemable for goods or services by the Provider. The Provider, not BidOnHealth.com, is the seller of the Voucher and the goods and services and is solely responsible for redeeming any Voucher you purchase. BidOnHealth.com sells a Voucher that can be redeemed in connection with your purchase of the goods or services from Provider.

Terms and Conditions for Vouchers are as follows:

  • Provider Voucher may be applied only to merchandise or services sold by Provider.
  • Limit one (1) Voucher per redemption. Only one Voucher can be used per order unless otherwise specified by Provider.
  • All Vouchers require a doctor’s prescription. Even if you purchase a Voucher, you will only be able to redeem it if you have a valid doctor’s prescription. By purchasing a Voucher on the Site, you represent that you have a valid prescription.
  • The issuing of credit is at the sole discretion of the Provider unless otherwise required by law.
  • Neither BidOnHealth.com nor the Provider is responsible for lost or stolen Vouchers or a Voucher’s reference number.
  • Vouchers cannot be combined with any other vouchers, third party certificates, coupons, or promotions, unless otherwise specified by Provider.
  • We make no representations about how your purchase or redemption of any Voucher will be treated by any health insurance company or other third-party payor. You may need to contact your insurer to see that amounts you spend on a Voucher are counted toward any deductible you may have. We cannot assist in communications with any third-party payor of yours.
  • Reproduction, sale or trade of any Voucher is prohibited unless this restriction is prohibited by applicable law, and said reproduction, sale or trade is done in compliance with applicable law.
  • Any attempted redemption not consistent with these terms and conditions will render the Voucher null and void.
  • Any transaction on the Site or involving any Voucher is void to the extent prohibited by law.
  • If a Voucher has a face value and you redeem the Voucher for less than its face value, you will only be entitled to a credit or cash equal to the difference between the face value and the amount you redeemed from the Provider if required by law. You will only be entitled to a continuing redemption value as noted above if the amount that you paid for the Voucher exceeds the amount you redeemed. For example, if you paid $20 for a Voucher which entitles you to purchase $50 of product by February 1, 2010 and you make a purchase for $40 on January 30, 2010, you will only be entitled to a credit or cash equal to the difference between the $50 face value and the amount you redeemed from the Provider (i.e., $10) if required by applicable law. You will not have any redemption value either because the amount you redeemed is more than what you paid for the Voucher.
  • All Vouchers are subject to the terms and conditions of BidOnHealth.com and the participating Restaurant or Provider. The Provider or Restaurant are the sellers of the goods or services which you are purchasing.
  • The holder and issuer of a Voucher is the Provider. As a holder and issuer of the Voucher, the Provider shall be fully responsible for any and all injuries, illnesses, damages, claims, liabilities and costs suffered by or in respect of a customer, caused in whole or in part by the Provider, as well as for any unclaimed property liability arising from unredeemed Vouchers or portions thereof. You waive, and release BidOnHealth.com and its officers, directors, employees and agents from, any claim, liabilities, damages, or injury arising from or related to any act or omission of Provider in connection with a Voucher or the services/goods provided in connection therewith and/or as it relates to compliance with applicable unclaimed property and other laws relating to the redemption of the Vouchers or any portion thereof. Vouchers are redeemable in their entirety and on a one-time basis only and may not be redeemed incrementally.
  • Under applicable law, the Provider may be responsible for allowing you to redeem your Voucher for the cash value based on the money you actually paid for your Voucher (i.e. if you paid $20 for a Voucher which gives you $50 of value to the Provider, the cash value that you paid is $20, not $50), for a period of time that extends beyond the expiration date on the Voucher. While the expiration date on the Voucher dictates the last date that you can use your Voucher at Provider for the promotional offer stated on the Voucher, applicable law may provide that the Provider is responsible for honoring the cash value that you paid for your Voucher for a period of time beyond the expiration date stated on the Voucher. In other words, you should be allowed to redeem the cash value (or purchase price) of your Voucher up until the later of: (1) the Voucher’s expiration date; or (2) the minimum length of time allowed by applicable law for a Voucher to expire. In the event that you have an expired Voucher and would like to redeem it for the price you paid to acquire it, please confirm that applicable law entitles you to such redemption, then contact the Provider.

7. Data Tracking

Cookies. To facilitate and customize your experience with the Site, we store cookies on your computer. A cookie is a small text file that is stored on a User’s computer for record-keeping purposes which contains information about that User. We use cookies to save you time while using the Site, remind us who you are, and track and target User interests in order to provide a customized experience. Most browsers automatically accept cookies, but you may be able to modify your browser settings to decline cookies. Please note that if you decline or delete these cookies, some parts of the Site may not work properly.

Other Tracking Devices. We may use other industry standard technologies like pixel tags and web beacons to track your use of our Site pages and promotions, or we may allow our Third Party Service Providers to use these devices on our behalf. Pixel tags and web beacons are tiny graphic images placed on certain pages on our Site, or in our emails that allow us to determine whether you have performed a specific action. When you access these pages or open or click an email, pixel tags and web beacons generate a non-personally identifiable notice of that action. Pixel tags allow us to measure and improve our understanding of visitor traffic and behavior on our Site, as well as give us a way to measure our performance. We may also utilize pixel tags and web beacons provided by our affiliates and/or marketing partners for the same purposes.
